School Paperwork requirements for Families currently enrolled in the 2023-24 school year:

All PreK-8th Grade Students must have their paperwork completed before the first day of school.

Back to School Paperwork:

All students in PreKindergarten,
Kindergarten,
7th Grade & NEW FAMILIES:

1. A certificate of immunization (MICR record)  OR a signed waiver stating immunizations are not being administered due to religious, medical, or other reasons. The signed waiver can be obtained at the Health Department, but you must schedule an appointment. 2. A Health Appraisal (click for pdf) (physical evaluation) performed within the preceding year signed by a licensed healthcare provider.  

Your family physician will be familiar with these requirements, and can help you update your immunization status and administer your child’s physical exam.

All New Families also need to submit:

1. A copy of your child’s birth certificate (does not need to be a certified copy).

2. Print, fill out, and return the Consent Form for Disclosure.

3. Print, fill out, and return the Release of Records Request
