25-26 Back to school paperwork

All PreK-8th Grade Students must have their paperwork completed before the first day of school.

We are so excited to have you back at school this year! We are implementing a new administrative system some of your school forms will look a little different. This new system will also be used for billing! Updates on that are coming soon from our new Business Manager, Karline Jensen. Below are the back to school forms that must be filled out by the start of school.

• Student Paperwork (completed electronically)

• Early Childhood Permissions

• Health Appraisal (completed and signed by your physician)

• Immunization Documentation or Waiver (provided by your local health department or physician)

  • An immunization waiver can be obtained by following the instructions on this website: https://www.gtcountymi.gov/530/Immunizations

• Extended Day Sign-up (for those that would like to participate in our after school childcare program)

• Sign up for Child Care Days (for those that would like to participate in our additional child care days)

• Copy of Birth Certificate (for all new families)
