Back to School Forms 2019-20

For your convenience, we have created a simple, online Back to School paperwork form for your family. Please COMPLETE THIS INFORMATION BY AUGUST 15TH . Also note that some families have experienced issues completing these google forms on iPhones. If possible, we recommend completing this paperwork on a desktop/laptop computer.

NEW STUDENTS
Please download a copy of the following and email/fax/drop off at school. 

  • School Records Release Form - this will allow Pathfinder to request your student's records be transferred
  • Health Appraisal Form - to be completed by your doctor and brought in with either your immunization record and/or waiver forms signed by Health Department

 

1st-8th GRADE STUDENTS

 

3rd-8th GRADE STUDENTS | Fall Experience

  • Student Waiver for Leelanau Outdoor Center - please review and sign then return to the Main Office. This will be for the Fall Experience overnight at the Leelanau Outdoor Center  from September 20-22. 3rd-5th grade students only spend one night at LOC (Sept 21st). 6th-8th grade students will be at LOC for two nights. 
  • Adult Waiver for Leelanau Outdoor Center - this is for any parent chaperones for the overnights at Leelanau Outdoor Center
  • LOC Packing List - please download and use when packing

 

STUDENT SUPPLY LISTS:

*Please label all items brought to school with your child's name - the school day can be busy and it's easy to accidentally leave something behind during a transition; our staff are happy to help find and return lost items, especially when we know where they go! Our Lost & Found can be located at the Gymnasium main entrance, items left in the Lost & Found will be donated in December and June during the school year.*

Supply Lists are also emailed from individual classroom teachers - please see their email for a list most specific to your child's class.

PreK Student Supplies:

  • Full change of clothes in ziplock bag labeled with child's name [stored in cubby]
  • Small Rest Blanket and/or Comfort Item [stored in cubby]
  • Indoor Shoes or Slippers for the Classroom
  • Water bottle [labeled with child's name]
  • Backpack with shoulder straps
  • Gym Shoes [MWF students only - stored in gym]
  • Daily packed lunch & two snacks [please remember utensils if needed]
  • Daily clothing should be weather appropriate & able to get messy

Y5-K Student Supplies:

  • Backpack [easily carried by child]
  • Water bottle [labeled with child's name]
  • Full change of clothes in ziplock bag labeled with child's name [stored in cubby]
  • Full Day students may bring a small blanket and book/book bag for "Read & Relax" time
  • Indoor Shoes or Slippers for the Classroom
  • Gym Shoes [please label with child's name - stored in gym]
  • Daily packed lunch & two snacks [please remember utensils if needed]
  • Daily clothing should be weather appropriate & able to get messy
  • 1 Pocket Folder - any design (K only)
  • 1 Box of Markers - any size (K only)

1st-4th Grade Student Supplies:

  • Backpack [easily carried by child]
  • Water bottle [labeled with child's name]
  • Indoor Shoes or Slippers for the Classroom
  • Gym Shoes [please label with child's name - stored in gym]
  • Daily packed lunch & snacks [please remember utensils if needed]
  • Daily clothing should be weather appropriate & able to get messy
  • #2 Pencils - sharpened if possible
  • Pocket Folders [please see email from teacher for specifics]
  • Spiral Notebooks [please see email from teacher for specifics]
  • 1 Box of Markers/Crayons/Color Pencils (optional - should fit in pencil case)
  • Hard Pencil Case (optional)
  • Personal Scissors [3rd only]
  • Personal 12" Ruler [3rd only]
  • Package of 3x5 note cards [3rd & 4th only]
  • 2 Packages of wide-ruled, 3-hole-punch writing paper [4th only]
  • 2 Packages of sticky notes [4th only]

5th-8th Grade Students:

  • Supply Lists for these grades will be sent and posted soon, stay tuned!

ATHLETIC REGISTRATION FORM - Registration will be emailed to you as soon as it is available. 

Must be completed each season for each student

  • Middle School Athletics = $ 90
  • Middle School Rowing = $75
  • Elementary Cross Country = $40
  • Norte = details coming soon
  • Elementary Tennis = detail coming soon


**All Middle School athletes must turn in an up-to-date Sports Physical to participate (physical form on file must be dates April 15th, 2018 or newer) **
